Tirzepatide is a dual receptor agonist that activates both GIP (glucose-dependent insulinotropic polypeptide) and GLP-1 (glucagon-like peptide-1) receptors. It's a modified 39-amino-acid peptide with a fatty acid chain (C20) that allows for albumin binding, extending its duration of action.
Mechanism of action:
- Stimulates insulin secretion (both first and second phases)
- Lowers glucagon levels, both in a glucose-dependent way

Dosing Guidelines:
- Start with 2.5 mg once weekly for the first 4 weeks (not for blood sugar control)
- Increase to 5 mg once weekly
- If needed, increase by 2.5 mg every 4 weeks based on response
Maximum dose: 15 mg once weekly
Missed dose:
- Take it within 4 days (96 hours) of the missed dose
- If more than 4 days have passed, skip it and return to the regular schedule
Changing dosing day: Must allow at least 72

- Do not freeze or use if frozen
- Protect from light
- Inspect visually for particles or discoloration before use
- Inject into abdomen, thigh, or upper arm; rotate sites with each dose
- When using with insulin, do not mix; inject at different sites
Note: The prefilled syringe contains natural rubber (latex derivative), which may cause allergic reactions.

- Discontinue immediately if severe allergic or hypersensitivity reactions develop
